Exclusions

The membership plan does not cover the following treatments and procedures:

  • Care Credit cannot be used to pay for the plan subscription or treatment.
  • Plan savings do not apply to certain procedures that are not offered at our practice. In this case, you may be referred to a specialist who does not participate in our plan.
  • Plan savings do not apply to the following procedures completed at our practice:
    • Orthodontics such as comprehensive orthodontic treatment and appliance therapy (CDT Code D8000-D8999)

Disclosures

  • Membership plans are not (i) insurance, (ii) qualified health plans under the Affordable Care Act, (iii) pre-payment for future services, or (iv) payment for access to discounted services. Rather, Membership plans are direct payment arrangements between plan members and Providers for services, products and care provided.
  • Providers establish their own membership fees, treatment fees, and terms. Fees and terms may vary depending on Provider, services and products provided, and location.
  • Payment for treatment not included in the membership plan is due at the time of service.
  • The savings offered in the membership plan can not be combined with any other offers or discounts.
  • Membership plans run for 12 months and include an auto renewal feature. Plan members can opt out of the auto renewal at any time.
  • Annual payment for Standard Care: $480 for Stand Care (enrollment fee waived).
  • Annual payment for Perio Premium: $828 for Perio Premium (enrollment fee waived).
  • Monthly payment for Standard Care: $40/month with $160 enrollment fee.
  • Monthly payment for Perio Premium: $69/month with $290 enrollment fee
  • Enrollment fee refunded after completion of 12-month contract.
