Membership Benefits

The Membership Plan provides access to preventive dental services and discounts on additional treatments performed at our practice. Benefits begin immediately upon enrollment, with no deductibles, waiting periods, annual maximums, denials, or pre-approvals. This plan is not dental insurance and is not intended to replace insurance coverage.

MEMBERSHIP OPTIONS

Standard Care Plan: $40 per month
• Two (2) standard cleanings, typically every 6 months
• Two regular exams
• Two oral screenings
Perio Premium Plan: $65 per month
• Three (3) periodontal maintenance visits, typically every 4 months (or as clinically recommended)
• Three regular exams
• Three oral screenings
Benefits Included with All Memberships
• One routine set of X-rays
• One emergency exam
• 20% discount on dental treatment and products
• 15% discount on cosmetics, implant, and in-house specialist procedures
• 10% subscription discount for additional family members

Enrollment Fee: one-time enrollment fee of $160 is required to activate the membership. This fee covers administrative setup and initial preventive care benefits. The enrollment fee is non-refundable.

Cancellation: this is a month-to-month plan and may be canceled with a 30-day written notice. Routine cleanings, periodontal maintenance visits, exams, and all other membership benefits are only available while the membership is active and in good standing.

Disclosures

Members must have active membership at the time services are provided.
Membership plans are not (i) insurance, (ii) qualified health plans under the Affordable Care Act, (iii) prepayment for future services, or (iv) payment for access to discounted services.
Memberships represent direct payment arrangements between the Member and the Practice.
Payment for any treatment not included in the plan is due at the time of service.
Savings cannot be combined with other offers or discounts.
Fees and terms are determined by the Practice and may vary by provider or service.
Care Credit cannot be used to pay for the plan or related treatment.
Plan savings do not apply to procedures not offered at our practice. You may be referred to a specialist who does not participate in the plan.
Savings do not apply to orthodontic treatments or appliance therapy (CDT Codes D8000–D8999).

  After my teeth cleaning I was examined by Dr Suzman, who was professional and precise.  I had a stain on my front tooth that has bothered me for quite some time.
