The Membership Plan provides access to preventive dental services and discounts on additional treatments performed at our practice. Benefits begin immediately upon enrollment, with no deductibles, waiting periods, annual maximums, denials, or pre-approvals. This plan is not dental insurance and is not intended to replace insurance coverage.

MEMBERSHIP OPTIONS

• Standard Care Plan: $40 per month
  • Two (2) standard cleanings, typically every 6 months
  • Two regular exams
  • Two oral screenings
• Perio Premium Plan: $65 per month
  • Three (3) periodontal maintenance visits, typically every 4 months (or as clinically recommended)
  • Three regular exams
  • Three oral screenings

  • One routine set of X-rays
  • One emergency exam
  • 20% discount on dental treatment and products
  • 15% discount on cosmetics, implant, and in-house specialist procedures
  • 10% subscription discount for additional family members

Enrollment Fee: one-time enrollment fee of $160 is required to activate the membership. This fee covers administrative setup and initial preventive care benefits. The enrollment fee is non-refundable.

Cancellation: this is a month-to-month plan and may be canceled with a 30-day written notice. Routine cleanings, periodontal maintenance visits, exams, and all other membership benefits are only available while the membership is active and in good standing.
